Enrollment

The Enrollment Process consists of two steps:

  1. Submitting Registration information

  2. Payment of at least a $75 deposit (goes toward the cost of weekend)

You will not be considered enrolled until we receive both your registration information and deposit payment (You may call the office, 614-445-0352, with a credit card # or mail a money order or check to our office.)

Registration Process

Registration information may be submitted in any of the following three ways:

  1. Fill out the Registration  online and submit it over the internet......or
  2. Contact our Intake Administrator on 614-445-0352 and provide them with the necessary information by phone.

Payment of Deposit

You may then make your payment of the $75 deposit by cash, money order, credit or debit card. These are the only forms of payment accepted by Crossroads Recovery Services. We do not accept personal checks under any circumstances.

Any of the above listed forms  of deposit payments can be made in person directly to our Intake Administrator at

Credit card and debit card deposit payments can be made over the phone or in person.

Cashiers Checks, and money orders can be made out to Crossroads Recovery Services and mailed to

Payment of Balance

The total cost of the Driver intervention Program is $310.00 for a semi private room (2 men/women per room) or $415.00 for a private room.

The balance may be paid with any of the above payment methods that can be used to make your deposit, with the following exception.
Credit or debit cards are not accepted at the program site for balance payment. If you wish to pay the balance due with a credit or debit card it must be phoned in or paid in person before the day of the program.
